Preventing Shoulder Injuries - A Physical Therapists Perspective

The shoulder is a fascinating joint. It allows us to manipulate objects in an infinite number of directions at various speeds. The shoulder is the most mobile joint in the body and provides many degrees of motion. This freedom of motion allows us to reach high and low, far and near for objects in space. The large amount of shoulder mobility does not come without a price. Increase in shoulder or (Glenohumeral Joint) mobility leads to a decrease in the stability of the joint. When a joint is unstable, it is more likely to suffer an injury. The most common injury to the shoulder is from a repetitive movement on an unstable joint. This movement can vary from typing at a computer with poor posture to an unstable tennis or golf swing. Shoulder injuries are not discriminatory to a specific sport or action. They affect both male and females athletes and non-athletes alike. The following guidelines in this article will help to prevent the possibility of developing a shoulder injury due to overhead sports, poor posture and improper weight training. Anatomy
The shoulder complex is made up of four joints. The glenohumeral joint, acromioclavicular joint, sternoclavicular joint and the scapulothoracic joint. Aside from these joints, there are other anatomical structures that play a large role in normal shoulder function. These include the cervical spine (neck), thoracic spine (upper back), clavicle and ribs. If any one of these structures is not functioning properly, they will have a direct negative impact on the shoulder complex. Full shoulder flexion in the saggital plane in a normal adult is 180. This movement is not accomplished by the glenohumeral joint alone. In fact, full arm elevation involves thoracic extension, the 1st & 2nd ribs must depress and move posterior, the scapula (shoulder blade) must rotate upwards, the clavicle needs to elevate and roll back and finally the glenohumeral joint needs to glide downward and roll upward. Mechanism of InjuryIf any one of these movements does not occur, the shoulder joint is susceptible to injury. The most common area of injury to the shoulder occurs in what is known as the sub-acromial space. This space is located just under the acromioclavicular joint. There are many structures passing through this space leading to a very tight passage. Included in this space is rotator cuff tendons, neurovascular bundles and other connective tissues. If any one of these structures is irritated or injured, inflammation occurs. Inflammation will take up valuable real estate in the sub-acromial space leading to possible impingement of the tendons, nerves or vasculature. The Rotator Cuff Muscles play a vital role in shoulder stability and injury prevention. The job of the four rotator cuff muscles (infraspinatus, supraspinatus, teres minor & subscapularis) is to synergistically contract at the same time to hold the shoulder centrally in the glenoid fossa to maintain an precise axis of rotation. If any one of these muscle is weak or injured, the shoulder will migrate out of position and either elevate or move forward in the joint capsule. This movement reduces space in the sub-acromial region which often leads to rotator cuff impingement. The rotator cuff tendons rub against the acromion causing friction, inflammation and eventually will tear if not resolved. The three rules of prevention. 1. Proper posture 2. Good body mechanics 3. Well rounded exercise program We are a product of our environment. A normal day for someone consists of the following: Wake up and head down to the breakfast table. We may sit for 20 minutes eating breakfast. Shower, get in the car or train and sit for a daily commute to work. Many people are sitting at a desk working on a computer for at least 5 hours daily. Then we get back in the car for the commute home. Dinner time arrives and again we sit down at the table for an hour. Following dinner maybe we sit and watch television or sit reading a book. Do you see a common theme here? Most of us are sitting all day long.When positioned in a seated position, gravity works against us to pull our head and shoulders forward. This posture leads to a decrease in sub-acromial space and places us at risk for shoulder impingement. How can this be avoided? Having a good ergonomic chair that places the hips slightly above the knees will orient the back in a good position. Arm rests are important for taking pressure of the shoulder girdle. The neck should be positioned in neutral by tucking the chin back in an attempt to lengthen the neck. The shoulders should be positioned down and back. Try to imagine bringing the shoulder blades toward the opposite back pocket. A stretch should be felt across the chest and shoulders. Body Mechanics Lifting, throwing and exercising using proper positioning can make a big difference in preventing injury. When lifting something overhead, the shoulders must stay down. If the shoulder moves upward towards the ears, you are increasing risk for impingement by closing down the sub-acromial space. Repetitive movements using improper body mechanics leads to shoulder injury. If an athletic movement such as the golf swing or tennis serve leads to pain in the shoulder, dont try to work through the pain. Seek professional advice from a physical therapist or a sports doctor. You may avoid a serious injury by inquiring when the discomfort first begins. Waiting until you can't bear the pain usually means the injury is serious and may require surgery.
Shoulder Exercises Exercising the shoulder to prevent injury should be conducted three times per week. There are many variations of exercises that can be used to strengthen the rotator cuff muscles and back musculature. Seated rows and lat pull-downs are great ways to strengthen the muscles around the shoulder blades. Keeping the shoulders down and pinching the blades together is the proper mechanics during this movement. Shoulder external and internal rotation will keep the infraspinatus, teres minor and subscapularis strong. Shoulder elevation to the front and side will strengthen the deltoids & supraspinatus. Make sure the shoulders do not hike up toward the ears with this exercise. Finally, proper stretching to keep the shoulders and pectoral muscles loose is vital to healthy shoulders. A door or corner stretch is great to open up the thoracic spine. This is accomplished by placing both forearms along the frame of the door with the elbows at shoulder height and keeping the abdominals tight. Gently bring the chest forward into the door until a comfortable stretch is felt across the chest and shoulders. Do not bounce or arch low back. This stretch should be held for a minimum of 30 seconds. In summary, proper posture and body mechanics is vital for preventing shoulder injuries. A well balanced exercise program that promotes flexibility, rotator cuff and back strengthening and precise body mechanics during exercise will allow persons to excel in their respective activities minimizing the risk for shoulder injury. I Can't Lose Weight, Because-

Never quit. - Bear Bryant I don't know if Alex Zanardi has ever heard of Bear Bryant, but I can't think of anyone who personifies this quote more. Never heard of Zanardi? He is a race car driver, a former champion in the CART series, and an athlete who doesn't know the meaning of the word quit. On September 15, 2001 Zanardi was doing what race car drivers do, he was racing the American Memorial 500 near Brandenburg, Germany. Leading the race with 13 laps to go, he had made his final pit stop and was attempting to merge into traffic when he spun into the path of another driver. Unable to avoid the spinning car, his fellow competitor sliced Zanardi's car in two, claiming Zanardi's lower legs in the wreckage. Alex Zanardi almost didn't survive the crash. His heart stopped beating seven times as doctors saved his life but amputated both legs, one above the knee and one below. Zanardi's career as a race car driver was over. But was it? Fast forward to 2003 and where do we find Alex Zanardi? Back in Germany, back in a race car, finishing those 13 laps that cost him his career and almost his life. Not content to just be there, he was driving the car at speeds approaching 200 mph. Fitted with prosthetic limbs and in cars with hand controls, he has not only been competitive but has won on the World Touring Car Championship circuit, and has tested F1 cars for BMW. So what does this have to do with losing weight and getting fit? I can't help thinking of Alex Zanardi when things aren't going quite right with my training, or my nutrition plan isn't helping me lose bodyfat as fast as I want, or I am tempted to stay in bed instead of going to the gym. Keep Life Simple-Keep Nutrition Simple, Part II

By Phyllis Ward In part 1, Keep Life Simple-Keep Fitness Simple, we talked about the basic principles of weight training. Proper form.
Switch your workout plan every 4-6 weeks. More often if you are an experienced weight trainer.
The basic Keep Life Simple exercises are; bench press, lat pull downs, lunges, squats and crunches
No more than 60 seconds rest between sets unless youre a power lifter.
Use enough weight. In general 8-10 reps to build muscle. For toning 12-15 reps. Youre already doing everything right so whats the problem? Wheres my six pack of abs? Could it be what youre eating? Did you know that about 80% of the results you want to see in your body have to do with the food you eat? Sure, the muscle is there but, we need to eat properly so that we can reveal the muscle that you have worked so hard for. Lets see; should I try the Grapefruit Diet, The Atikins Diet, The South Beach Diet, The Cabbage Soup Diet, The Zone? Should I go high protein, high carbs, no carbs, only eat low glycemic index? Do you see how complicated we can make eating just with the few diets Ive listed? Personally, Im waiting for the potato chips, chocolate chip cookie and ice cream diet! Believe it or not eating is easier than you think. God created a wonderful variety of fresh foods for us to eat. In general if it comes in a box, dont buy it. If it flies, swims, grows on a tree or in the ground, thats what you should be eating. Heres how really simple this is. Im going to give you a chart of foods you should never eat and a chart of foods you should always eat. Ice cream Oatmeal (or other whole grain cooked cereals)

Sugary breakfast cereals Fish and shellfish Some of you like to have a lot of choices of what to eat and others, well, too many choices become dangerous. It doesnt look like you have a lot of things to choose from but, think of how many types of fruits and vegetables there are. Keep it simple and you cant go wrong. There is so much information out of nutrition and what a portion is but just in case youre not sure, heres the deal; Three ounces of meat is about the size of the palm of your hand. A serving of fruits, vegetables, rice, etc is about a half cup. Thats about the size of what you can hold in a cupped hand. If you go to a fast food restaurant and they ask you would you like to supersize your meal? Just say NO! If you are a 350 pound linebacker, or a competitive athlete, those figures are going to vary but, for the general population those figures are going to work. How do you tell if youre eating the right amount of food? Again, keep it simple! How your body looks and how your clothes fit are still 2 of the best ways to see whats working for you. You can also measure body fat, weigh and take measurements. You know that were going to have to bring up the w word. Yes, water. Water is often over-looked as a nutrient. People who do not drink enough water often feel lethargic; have headaches, muscle aches and cramps. Many people do not know how much water they really need during a day.
How much water you need depends on your activity level, how many foods that you eat that are high in water content and if you drink alcohol, caffeine and your body weight. A good rule of thumb is the traditional 64 ounces per day. Thats 8-8 oz. glasses of water. Personally, I like to get in 70% ounces to pound in body weight. If a person weighs 130 pounds, 70% of 130 pounds would be 91 ounces. If you are very active or drink a lot of coffee, you will need more water. Coffee and alcohol do not count as fluids in your diet. I actually had a client ask me if the water in his scotch and water counted toward his fluids for the day. The answer is.NO!!!! Water is one of the most important elements of the human body. It makes up approximately 70% of our muscles, and about 75% of our brains. It is also an essential part of a healthy diet and plays a vital role in the proper functioning of the body. Acts as a lubricant for the body, moistening eyes, mouth, nose, and skin
Helps maintain adequate blood volume
Helps regulate body temperature, especially in warm weather
Helps prevent constipation

Acts as an appetite suppressant
Is calorie-free
Boosts metabolism when drunk cold (we burn 2 calories per glass as we warm it up to body temperature) Be sure to reward your body with healthy food. Dieting Strategy -- Stop Diet Drop Out

To stop a diet plan before reaching your goal or before reaching a pre-defined time limit is called diet drop out. Not one diet succeeds at keeping diet drop out rates low. One or more of the following strategies are used by diet plans in attempts to eliminate diet drop out:Shorter Dieting Sessions This is a strategy that more often back fires, setting the stage for weight gain. Subjects placed on shorter dieting sessions experienced weight loss at a faster rate than those following longer sessions. Fast weight loss (over 3-4 pounds/week) is unhealthy and negates any progress made on lowering diet drop out rates. Severe Calorie Restriction This strategy follows the same principle as faster dieting sessions. Allow dieters to lose weight fast, and they won't have time to drop out. Unfortunately, severe calorie restriction also causes weight gain. Diet Holidays This strategy allows dieters to take long breaks or "holidays" from following the diet. However, the opposite result is often observed. Diet drop out rates increase because many dieters never start the diet again. In order to provide a strategy that works, diet plans should ask dieters why they drop out. Then simply provide a solution for the reason dieters drop out, problem solved. Why Dieters Drop Out When dieters are asked what is the hardest part about dieting, the response is surprising. Most dieters say that following strict diet menus, not exercise, is the hardest part. The Other Side of the Stethescope

In his professional life Dr. Ruiz was a New York City physician on the cutting edge of detection and treatment of malignant melanoma, the most serious form of skin cancer. He had recently flown to Florida to visit family hospitalized there and had entered the hospital unchallenged without even identifying himself when he entered the intensive care unit in Orlandos most prominent hospital. In Florida he had seen children running up and down the hospital halls and family visiting anytime they chose. Far from strictly enforced, in Florida families challenged the authority of nurses and doctors to restrict visitation even to allow for patient recovery. Further, Dr. Ruiz had seen nurses publicly reprimanded by supervisors for enforcing visitation policies in Florida. He was seeing that the All Hazards preparedness and Situationally Sensitive Security to which he was accustomed to was far from universal. A year earlier, at age 39, Dr. Ruiz suffered a heart attack while working in New York City. Like so many heart attacks in the north, his began while shoveling snow and ended on the operating table. Owing to his young age and the fact that he exercised daily, he did well and went home. His story would have ended there except Dr. Ruiz realized there was a stark difference between his reality and the state of healthcare safety in the rest of the nation. Dr. Ruiz had always been one of those doctors who never saw the need for increased disaster preparedness for healthcare. Practicing in New York City after 9/11 it seemed to him that every hospital and healthcare facility had instituted Situationally Sensitive Security and All Hazards Disaster Plans. Despite the fact that he had family and friends involved in national preparedness he had always assumed that every facility was as ready as the ones where he worked. Prior to his heart attack, Dr. Ruiz had never walked in the front door of the hospital. When he arrived as a patient, he entered the front door and was immediately asked for photo id. He showed his drivers license and his wife was immediately stopped and asked for her id. For the first time he saw that no one entered the hospital without scrutiny and business at the hospital. This was such a contrast to what he now saw in Florida. Could it be that the rest of the nation was this unsafe? Once admitted to the hospital, Dr. Ruiz learned that visiting hours were not only defined and restricted, but strictly enforced. Moreover, small children such as his own could not visit on the patient floor; he would have to be well enough to visit with them in the family spaces. What a difference from the world he now saw! How do the doctors and nurses work in such a place? When Dr. Ruiz returned home he decided to see if his perceptions were in fact correct. He again entered through the front door. Had it not been for his hospital id, he would not have gotten in. He learned that on this day there had been an incident at another hospital and the hospital had increased the level of security. For the first time he took note of the attitude and decorum of his own patients visitors. In sharp contrast to what he had seen in Florida, these New York visitors listened to instructions, obeyed visitor policies and followed the instructions of the nurses. The Safe Work Environment What Dr. Ruiz came to realize is what preparedness experts have been saying for years; healthcare has few well prepared institutions while the majority of healthcare has chosen to ignore the threats and the most obvious solutions. The most important change is to incorporate security and preparedness into the daily regimen of every hospital function and every hospital employee. The Situationally Sensitive Security Dr. Ruiz encountered in his hospital ensures that the hospital staff as well as all visitors are accustomed to some level of scrutiny when entering the hospital. At the lowest levels, no more that an id check occurs, but as security concerns increase, the level of scrutiny and restriction increase. This type of daily routine ensures that when increased security is required, the baseline behaviors are in place and familiar. The same philosophy is the basis of Continuous Integrated Triage and several other All Hazards protocols. Workplace safety has become as much a component of All Hazards preparedness and patient safety initiatives. Facilities that have instituted this expanded approach to preparedness have found that patient safety initiative, employee safety programs and All Hazards preparedness are a natural combination. Funding once used for just one program can be applied to all three areas simultaneously thus allowing a hospital or healthcare institution to benefit in all three realms for each dollar spent. More importantly, Dr. Ruiz inadvertently identified the reason that many preparedness experts have failed to successfully persuade hospital and healthcare decision makers to spend money on preparedness. Keeping the Flame Alive on Your Wedding Anniversary

By Ian Tham Wedding anniversaries are special events. Especially those big ones that mark 10th, 20th, 30th, 40th and 50th anniverary .
On that special day, you can take time off to reminisce the past. You can take a look at your mementos starting from your first date. Do dust off the photo album and go through pictures of special occasions such as your wedding day or delivery of your newborn child. You can play the video of your wedding day or dress up in your wedding clothes. Maybe by now they are too small or tight for you.
On this day, you stop and reflect the state of the union between husband and wife. Then it hits you so hard that you have actually spent so little time on your dear spouse. Why? We have got to concentrate mind and soul on our careers and slog hard from morning to night at the office. By the end of the day, both partners are tired and too exhausted to talk to each other. Then your focus of attention will be on your kids. You have to take care of them play with them ,feed them, monitor them and supervise them . You have to check whether they have eaten , bathed and done their homework.
Besides work and family commitments, you have to worry about finance . Do you have enough money to pay bills ranging from housing, car loans, groceries, clothes, school fees? It's enough to kill the joy of marriage.
But on the other hand , you can reflect on the positive, such as overcoming problems and obstacles through strength and perseverance.
Both partners should share something in common. Shared experiences makes a lot of difference. Here are a few examples.Saving money to buy your first house. The two partners deciding where to live, and what kind of house to live in.
The couple can find delight in sharing tasks together. I remember taking care of my baby son. He would cry in the middle of the night wanting either to change diapers or to be fed. We would turn turns looking after baby. She would be tired out attending to his night time needs after one or two days. I would then take care of baby on the subsequent nights.
We go to church together to worship the Lord and it's such a wonderful spiritual experience every Sunday. We also go to the gym and workout together.
Both partners need to have same mindset or similar interest. Both my wife and I enjoy going to the movies, love children and enjoy traveling.
What similar interests do you have as a couple ? Find out what similar interests both of you have. Is it a particular kind of food, movies or travels?
And when baby comes along , make sure you also have some time to spend on your own. Ideally, you could get your parents or parents in law to help take care of baby.

What Types of Cosmetic Surgery Are Available

Well, you must change that attitude. You have to understand that by using the correct tool it can avoid you from hurting and damaging the project you are trying to repair. Well, it sure does. That means to see this tic toc between order and chaos in our lives and rather than fight it, actually manage it. Infatuation with order will breed huge chaos. Instead, we can say, oh, theres a challenge, where is the balance in that, oh theres a winner wheres the balance in that In the diagram above, we call it the consciousness cone, inspiration is at the top. You can see that a person who is at their highest ego state at the bottom is spending huge amounts of energy in emotion, running from very happy, to very sad. From infatuation to resentment. This is the lotto winner who, without the mindset to manage it, spends their winnings within months and ends up in more debt than they had before winning. Moderation is the keyword at the top, it is a focus on something more that wild upsides and deep depressive downsides. This is the middle path, where love and inspiration are a priority. Very few people can sustain this level of growth. They most often yoyo up and down the cone some days at the top, others at the bottom. Viewpoint is everything. The person who prioritizes their ego sees the world from their own circumstances and looks out at it. To them, when they look at the stars, they are at the center and the whole universe is rotating around them. To the inspired individual, they look at the stars and it appears they are on the outer looking in. The ego driven individual will take everything personally and become disturbed by the small things of life. The inspired individual see the balance in everything and takes nothing personally, to them, there is no self to take anywhere. To the inspired individual, there is no soul, there is no individuality to identify with, not even a good and bad to protect. To the inspired individual there is work and life and love and doing. The elations and depressions of it all are just seasons of the day, there is simply no need to react. The search for truth, is the search beyond self. That truth lies hidden below the ego. When the fog is cleared everyday life becomes transparent we see the beauty that lies beneath. One moment standing in the midst of natures orchestra with an open heart seems like a whole lifetime, if we are in tune with her rhythm. What can be learned from books means so much, yet, it is all someone elses, repeated, regurgitated, recapitulated, renovated, rejuvenated truth. We must learn to write our own books, instead of parroting what others have said, and in this, let nature be our guide The ground is steep, the rocks are wet, we must take care of each other. There is no alone, with Inspiration, one is never alone. Every tree is a sacred friend, every footstep divine, the rocks are mates ready to lend a hand. Inspiration teaches us friendship with nature. We learn to be humble to the power that is greater than ourselves, not because of fear or faith, but through trust in the heart of the divine. Nature is our witness. Everyday inspiration does not come from sitting in caves with crossed legs, (although thats how Chris found it) it comes from life and an understanding of it. Whether we are fishing off the pier or swinging a billion dollar deal, singing in the shower or to a million people, we see beauty, we see life and we are in awe of it. The first step is to unlock what has already been learned and then unlearn it. This unlearning is what is called the gaining of real wisdom. In the past you may have said that person is bad. This is knowledge taken from our learning, but such learning and judgment are in defiance of natural law because the experience is one sided. It is out of balance, and all emotion including your emotional downers comes from imbalanced thought. Emotional downers is the product of an imbalanced thought about something or someone in the past. Unlearning is to see the circumstance differently. There is always two sides to that person or situation, you just never saw it. When you see the goodness in a situation you have already called bad, you have unlearned; you have seen with two eyes. You learn by seeing one half of the story; then unlearn and see two sides. This makes the learning complete. Natures law pervades every walk of life so it is not specifically what you think that matters, it is how you think that makes the difference. You must change how you think from one sided learning (causes depression) to balanced thinking (causes love). S came to me for a consult. She was 18 but looked 12. For nearly 5 years she had been diagnosed as depressed and due to that had taken on board some eating disorders. Her mother was with us in the consult and was obviously shattered by her daughters declining health. Eating disorders that deprive the body of nourishment are self deprecating, something about S, was so unacceptable, in her opinion, that she wanted no body to live in, she was trying to eliminate her physical presence on earth (bulimia or anorexia). In S case anorexia because she was not vacillating between infatuation and resentment. She was only conscious of her depressions. Emotional downers, looked at from the point of view of natures law, is an obsession with a self righteous set of beliefs that are stubbornly held in the individuals mind against the backdrop of everyday life. Remember, there are two sides to everything, Expressed and Repressed. The expressed disappointment with life is the balance to the repressed belief that the depressed person knows what is best for themselves and others. So there were two issues. One a thought that S was holding onto that was causing her to become depressed. The other was nature destroying her body, which meant she was not fulfilling, or had already fulfilled her purpose. Me so if thats what you think about the real world, that means in your fantasy you believe you are better than everyone else but no one recognizes it

Me when we have such inspirations about ourselves its usually because something happened really painful and we create a fantasy of ourselves inside our mind. Then when we cant live that fantasy, we get angry at the world and want to go somewhere where our fantasy can become reality

S heaven To extract the gift from our life path, we must get beyond the ego, and to do that we must become an empty cup. This means letting go of and changing some of your judgments about people in the past or present. Often a person, like P, comes to me and says "here I am. Can you help me spiritually?", to which I answer, "yes". Then they say "I want to know what you think about life and death, or about the beginning and end of existence". This prompts me to wonder what their attitude will be if their existing opinion does not agree with mine. They seem to want to learn, yet they do not want to be empty of their preconceptions and expectations. That means going to the stream wanting water, but keeping one's cup covered up, A full cup is the ego, jammed full of beliefs, identities and ideals which prevent change. They are learned ego responses that have served you in the past, but maybe blocking you now. They maybe blocking your heart from being open, but instead of changing the thoughts and ego attachment people will change their relationship instead. This is a high cost, to keep infatuating the ego. With emptiness, comes an innocence that is open to change. With that emptiness comes trust to open and let go, find love and the recognition that there is an order in each chaos and we do not need to fight. P came to me really depressed. I have had cancer 10 times and now its 11. I want to know if there is anything I can do to stop this
